Organization and Hazing Violations - Campus Hazing Transparency Report

Collin’s Law and the federal Stop Campus Hazing Act require universities to list hazing violations before the first day of January and August each year. To promote transparency, the university does not wait to publish violations. This page, which contains violations for the preceding five years, is updated with new violations once the adjudication and appeal process under the Code of Student Conduct has been concluded. Hazing violations for individuals are included, but information shared may be limited or redacted to protect personally identifiable information for Ohio State students. Additionally, this page contains information about all organization violations, not just hazing, for organizations on all Ohio State campuses.

Regarding the dates listed in the report:

  • Incident date: this is the date on which the incident was alleged to have occurred.
  • Investigation initiated: this is the date on which the university’s investigation was initiated, and notice was first given to the organization that it was facing potential charges for hazing or other violations
  • Charge date: this is the date the university concluded its investigation and issued formal charges to the organization
  • Resolution date: this is the date on which the university provided written notice to the organization that a hazing (or other) violation(s) has been determined and that the university has issued sanctions

Zeta Phi Beta Sorority, Inc. - Hazing

This chapter engaged in or attempted to engage in conduct that included the following over a period of three months:

  • Requiring new members to participate in an “underground” experience as part of their new member process
  • Requiring new members to constantly participate in late night activities that lead to sleep deprivation
  • Requiring new members to participate in mandatory exercises, such as planks, wall sits, pushups, and running
  • Requiring new members to follow special rules, such as not being allowed to ride the bus, not being allowed to touch the grass, not being allowed to enter certain buildings, not being allowed to wear make-up, not being allowed to drink alcohol, not being allowed to have sex, required to wear hair down, and required to report to locations on demand
  • Requiring new members to participate in acts of servitude, such as bringing food to active members of the organization
  • Verbally assaulting and threatening new members
  • Physically assaulting new members to the point of injury with the use of a paddle

Revive Students

The Revive student organization engaged in or attempted to engage in conduct that included:

  • Holding an event past the reserved time of 9:00 pm 
  • Having an event with amplified sound that went past 10:00 pm, which a staff member reported as being audible inside a residence hall 
  • Utilizing five 100-gallon water tanks after a staff member instructed in an email they were not to be used, and after staff members gave additional verbal instruction that the water tanks were not to be used and should be emptied 
  • Accessing the Environmental Services (EVS) closest in Blackburn Victory, which had not been approved as part of the event registration 
  • Accessing water from the EVS closed in Blackburn Victory by bringing a hose to fill up the five 100-gallon water tanks, which had not been approved as part of the event registration 
  • Utilizing a pop-up tent that had not been approved as part of the event registration 
  • Utilizing electrical cords across sidewalks that were not covered, causing a tripping hazard 
  • Advertising the event with signs taped on lamp posts across campus 
  • Utilizing yard signs that were staked into the ground

From spring 2022 onward, the Zeta Nu chapter of Phi Chi Theta engaged or attempted to engage in conduct that includes, but is not limited to: encouraging new members to complete around 10 hours of coffee chats per week (new members shared that these chats created struggles around time management and stress); asking only new members to dress in business casual attire on Thursdays from 9 a.m. to 5 p.m. (active members have been encouraged to "snitch" on new members who did not dress in business casual on Thursdays, resulting in a discussion with the pledge educator); using an "unofficial" group chat where members discuss alcohol, party information, how to get around the legal drinking age while at bars, and create groups (including underage members) for drinking events like keg races and/or beer Olympics; hosting parties after the organization's initiation banquet and making alcohol available to all attendees with no policy or practice of checking identification/age; providing alcohol at the spring 2022 social

Omega Gamma - Hazing

Description: 

Students pledging Omega Gamma, a service fraternity, were led to an off-campus house on a scavenger hunt that was part of an initiation ritual. This took place after the initiation ceremony at a building on campus. The pledges' phones were taken away and they were told as a group they needed to drink 50 alcoholic drinks to get their phones back. The organization took responsibility and requested an administrative decision.

It was reported that during the spring 2022 semester, the Alpha Tau Zeta chapter of FarmHouse held initiation events during Builder of Men week which included an overnight component that ran from approximately 4 p.m. through 7 a.m. Additionally, alcohol was offered to new members during Builder of Men week, including the “breakfast beer” toast. New members were charged with the care of two ducks for a secret Builder of Men week tradition, which included providing shelter, water, food, and time to care for animals.

Description: 

It was reported that on or around March 14, 2022, in connection with an out-of-state trip by seven members, three guests, and one new member, and on other dates during the spring 2022 semester in Columbus, Ohio at the chapter house or other locations associated with members, the chapter engaged or attempted to engage in conduct that includes, but is not limited to:
(1) Doing, requiring and/or encouraging members or new members, regardless of their willingness to participate, in conjunction with continued membership and/or participation in the fraternity, to: (a) consume alcohol in a forced or coerced manner, including, but not limited to, consuming various types of alcohol out of a funnel or beer bong, playing drinking games and/or consuming a specific amount of alcohol during a specific timeframe, (b) engage in embarrassing activities, including, but not limited to, members and/or new members doing "naked laps" and new members being asked, encouraged and/or coerced to wear specific clothing or uniforms, (c) engage in assigned duties and/or pointless tasks, such as posting specific content in group messages, collecting signatures from current members and/or cleaning the chapter house or other locations associated with members, (d) do calisthenics, such as doing push-ups if a member steps on the fraternity's letters in the chapter house. 
(2) As it relates to the purchase, consumption and/or distribution of alcohol, requiring and/or encouraging current members or new members - without regard to whether or not they were 21 years of age or older - to: (a) consume alcohol that was provided by the chapter and/or its members using voluntarily collected dues of members, including by those who were underage (b) participate in events that include alcohol as part of a competition, including, but not limited to, playing drinking games and/or chugging or rapidly consuming alcohol.

Description: 

It was reported that, during the autumn 2021 semester, the Alpha chapter of Sigma Alpha encouraged/required new members to stay at their Bigs' houses during Inspiration Week where their phones were taken and they could not be on social media. New members were not allowed to go anywhere without an active member being present with them. The Monday night of Inspiration Week, new members were required to stand in front of active members to recite information while receiving constructive feedback. New members became upset during this event and described this experience as humiliating, overwhelming or stressful. The Tuesday night of Inspiration Week, which involved the agricultural fraternities, new members were told to walk around a house and be in rooms with their eyes closed or backs to individuals in the rooms. New members had to guess who was present in the room from clues given and/or asking yes or no questions. When new members guessed incorrectly, multiple active members made demeaning or rude comments.

During the spring 2021 semester, from January 22, 2021, to April 5, 2021, the Waw chapter was under an interim suspension and was not permitted to engage in any activities. The recruitment and interactions with the new member class in the spring 2021 semester failed to comply with the interim suspension. Additionally, members were encouraged/required to: deliver food and drinks to active members, clean the house, set up for events and move furniture.
