Student Mediation Program Referral Form

You may request a mediation session or refer a conflict you think may be ideal for mediation to the office in one of several ways.

1.  Call 614-292-0748.

2.  Stop by 33 West 11th Avenue, Room 115, Columbus, Ohio

3.  Email studentconduct@osu.edu

4.  Complete the Student Mediation Program Referral Form. Email it to studentconduct@osu.edu or fax it to 614-292-2098, and we will get back in touch with you promptly.

 

After Referral is Made: The Mediation Process

The office will notify the student(s) involved and request their attendance at a mediation. Since mediation is voluntary, all parties must be willing to consider a mediation in order for the process to continue.

The mediation will occur in a neutral location on campus. The mediator will advise the parties of the mediation rules and procedures beforehand. Regardless of whether an agreement is reached, the office will follow up approximately six weeks later to ask for feedback on the mediation session and the agreement, if applicable. The office must approve all agreements to ensure compliance with university policies.
